Frequently Asked Questions
The HCN was established by Virginia legislation (§ 32.1-372) in 2017. Hospitals began onboarding in 2018, and participation has grown each year since to include additional provider groups and care team members.
All Virginia hospitals, as well as many health plans and other organizations providing patient care, are already participating in the HCN. If you work for an organization that already participates and would like to gain individual access to HCN features, or if you are unsure whether your organization is participating, please email HCNsupport@vhi.org.
The HCN is supported through public and private funding. Cost to participate is based on organization type and selected program features.
Many organizations can participate at no cost. Submit a discovery form to learn more.
All participating organizations have access to custom alerts/notifications and a web-based portal. Organizations paying a per-member-per-month fee can also access curated admission-discharge-transfer feeds as well as an upgraded version of the portal.
The following criteria can trigger a custom notification to a participating care team member or an alert on an emergency department trackboard:
Core Criteria:
- High Utilization: 5+ ED encounters within 12 months
- Rising Risk: 3+ ED encounters within 90 days
- All ED Encounters: Any emergency department encounter
- All Inpatient Encounters: Any inpatient encounter
- All SNF Encounters
- MDRO Lab Results
- PMP NarxScore ≥500,
- Advanced care planning documents
- Care insights or safety and security events documentation
- History of behavioral health or substance use disorder
- ED visit with mental/behavioral health diagnosis
- ED visit with suicidal ideation, suicide attempt and/or self-harm
- ED visit with opioid overdose
- ED visit with alcohol abuse
- ED visit with opioid use disorder
The HCN’s secure infrastructure meets all federal and state privacy and security regulations, including the Health Insurance Portability and Accountability Act of 1996 (HIPAA). Patient data within the HCN can only be shared with authorized providers for treatment, payment or healthcare operations (TPO) via HITRUST-certified and other secure health information technologies.
